Planning In Partnership PiP (Formerly Life Transitions for Seniors)

Planning in Partnership  (PiP), a free resource for Montclair residents over the age of 55, is a project funded by Partners for Health to help Montclair area residents through life changes associated with aging. PiP provides a case manager at no cost to Montclair area residents who might have questions about:

  • Retirement
  • Housing
  • Healthcare Challenges
  • Transportation
  • Loss of a Partner

If you believe that PiP offers services from which you may benefit, the following choices are available for you to start your conversation with Sarah Nikpour, our program social worker:

  • Private confidential meetings can be scheduled. You can call (201.463.4594) and either:
    • ask for an appointment with Sarah Nikpour or for more information. Private confidential meetings can be scheduled at the Montclair Township offices, 205 Claremont Ave, or at other convenient community locations.
    • or see Sarah during Drop-In Hours at the Montclair Municipal Building, 205 Claremont Ave. 3rd floor, Mon.-Tues. 10:30 am – 2 pm and Weds. – Thurs. 3:15 – 4:30 pm
